Sec. 3.

(a) The application for inclusion as an approved location on the health care volunteer registry must be made upon forms supplied by the board.
(b) Each applicant shall provide the board with evidence of compliance with the requirements of this rule.
(c) All information in the application shall be submitted under oath or affirmation, subject to the penalties of perjury.
(d) Application forms submitted to the board must be complete. All supporting documents required by the application must be submitted with the application.
