Section 844 IAC 4-6-4 - Mandatory renewal; fees

Sec. 4.

Each licensee shall submit a fee as determined by the medical licensing board of Indiana for each year or fraction thereof, in the form as provided under IC 25-1-8-2(d) payable to the order of the health professions bureau.
