Sec. 2.
(a) All holders of certificates as a certified dietitian shall notify the board of any change of address or name in writing within fifteen (15) days of such changes.(b) Before a replacement of a certificate or pocket card can be issued by the board by reason of a name change, notification of name changes must be received by the board along with a notarized copy of a marriage certificate, court decree evidencing such change, or Social Security card reflecting the new name. Replacements for lost, damaged, or stolen certificates or pocket cards shall be issued on written request to the board.
