Section 844 IAC 18-1-10 - Name change or change of address

Sec. 10.

(a) Each license holder shall inform the agency, in writing, of all changes in address or name within thirty (30) days of the change.

(b) A license holder's failure to receive notification of renewal due to failure to notify the agency of a change of address or name shall not:

(1) constitute an error on the part of the agency; or

(2) exonerate or otherwise excuse the license holder from renewing the license.
