Section 828 IAC 1-3-4 - Dental licensure by endorsement; failure to renew expired license

Sec. 4.

The holder of a license that has expired under IC 25-14-1-10 for failure to renew may not apply for licensure by endorsement under IC 25-14-1-16(b) and this rule if the holder of the license has practiced dentistry in Indiana during the period of time while the license was expired.
