Section 810 IAC 2-1-3 - Maintenance of the registry

Sec. 3.

(a) The registry shall be made available to the public on the Internet through the computer gateway administered by the office of technology established by IC 4-13.1-2-1, including each registrant's:

(1) name;

(2) city and state of residence;

(3) licensed profession;

(4) license number;

(5) date of registration; and

(6) date of expiration for the registration.

(b) Upon request, the agency shall have access to and copies of all documents listed in IC 34-30-13-1.2 that a practitioner used to establish immunity from civil liability in treating an individual.
