Sec. 6.
(a) A registration under section 2, 4, or 5 of this rule is valid for one (1) year.(b) A PEO shall complete a renewal application on a form prescribed by the department. The registration shall: (1) identify any change in the information provided with the PEO's initial registration; and(2) include an update of the PEO's list of Indiana clients. For a PEO registered under section 5 of this rule, the renewal request and any updated information may be provided by the approved independent national organization that provided the information for the initial registration.
(c) The renewal application shall be accompanied by a renewal fee in the amount of two hundred fifty dollars ($250) for any type of registration.(d) If the PEO maintains a health benefit plan that is not fully insured, it shall provide audited financial statements for the health benefit plan trust account with its renewal application. For a PEO registered under section 5 of this rule, the audited financial statements for any health benefit plan trust account may be provided by the approved independent national organization that provided information for the initial registration.(e) The renewal application with supporting documentation is due one hundred eighty (180) days after the close of the PEO's fiscal year. A late fee of two hundred fifty dollars ($250) will be assessed to any PEO that does not timely submit the renewal application and supporting documentation.
