Section 675 IAC 12-9-6 - Fireworks stand retail sales permits

Sec. 6.

An applicant for a fireworks stand retail sales permit shall submit an application to the state fire marshal on forms provided by the state fire marshal. Each application shall provide the following:

(1) The information required by IC 22-11-14-7.

(2) Evidence of incorporation under IC 23-7-1.1 as a not-for-profit corporation if an exemption from the permit fee is claimed.
