Sec. 2.
(a) An individual applying for a certification based on reciprocity must file an application on a form prescribed by the department.(b) An application must be filed for each certification based on reciprocity.(c) An application for a certification based on reciprocity must include the following:(1) Identifying information about the applicant.(2) Evidence the applicant has completed the same or substantially similar training and testing for the certification as that required by the education board, including: (A) a certification at the same level by the International Fire Service Accreditation Congress, National Board on Fire Service Professional Qualifications, or United States Department of Homeland Security; or(B) proof of successful completion of a certification program based on the same certification standards as the certification approved by the education board.(3) A sworn statement by the applicant affirming the information provided is true and accurate.(4) A nonrefundable processing fee of fifty dollars ($50) under IC 22-14-2-7(d).Board of Firefighting Personnel Standards and Education; 655 IAC 2-4-2; filed 8/1/2024, 4:17 p.m.: 20240828-IR-655240059FRA
