Section 460 IAC 7-5-1 - Sections of an ISP

Sec. 1.

An ISP shall have the following sections:

(1) Personal and demographic information section.

(2) Individual's diagnosis section.

(3) Individual's emergency contacts section.

(4) Outcome section.

(5) Statement of agreement section.

(6) Individualized support plan participants section.

(7) Meeting issues and requirements section.

(8) An optional attachment regarding resources.
