Section 460 IAC 6-3-52.1 - "Service planner" defined

Sec. 52.1.

"Service planner" means the worksheet approved by a BDDS service coordinator that outlines on a weekly basis:

(1) the services an individual is to receive; and

(2) the intensity of those services, including staffing levels, if applicable.
