Section 460 IAC 14-8-4 - Record of services

Sec. 4.

For every individual for whom a severity determination has been made, the individual's record of services must include the following:

(1) The level of severity of the individual's disability.

(2) All information obtained or generated for the purpose of determining the level of severity.
