Section 460 IAC 14-16-2 - Diagnoses, prescriptions, and recommendations for physical and mental restoration services

Sec. 2.

Diagnoses and prescriptions for physical and mental restoration services shall be obtained only from a licensed or certified specialist, including, but not limited to, the following:

(1) Licensed physicians.

(2) Registered occupational therapists.

(3) Licensed psychologists.

(4) Licensed optometrists.

(5) Licensed podiatrists.

(6) Certified speech-language pathologists.

(7) Certified audiologists.

(8) Licensed speech and hearing therapists.

(9) Licensed nurses.

(10) Licensed alcohol and drug addiction counselors.

(11) Licensed clinical social workers.

(12) Licensed physician assistants.
