Section 410 IAC 28-2-2 - Reporting

Sec. 2.

A physician who performs a surgical treatment for the treatment of morbid obesity shall do the following:

(1) Monitor the patient for five (5) years following the patient's surgery, unless the physician is unable to locate the patient after making reasonable efforts.

(2) Report before June 30 for the cases in the last six (6) months of the preceding year and December 31 for the first half of the current year to the department any:

(A) death;

(B) serious side effect; or

(C) major complication;

of the patient.
