Sec. 1.
(a) The center shall file an application for licensure on a yearly basis with the division, prior to the expiration of the current license.(b) A license is not transferable or assignable, and is issued only for the premises named in the application.(c) If multiple buildings or sites (contracted, rented, or leased) are licensed under a single license, the licensee shall provide for these buildings as a single integrated system as follows:(1) All buildings or portions of buildings under a single license must be governed by a single governing body and under the administrative control of a single chief executive officer.(2) All facilities operating under a single license must have a single medical staff.(d) All changes in ownership, name, and address must be reported in writing to the division.
