Sec. 1.
(a) The hospital shall file an application for licensure on a yearly basis with the division, prior to the expiration of the current license.(b) A license is not transferable or assignable and shall be issued only for the premises named in the application.(c) If multiple buildings are licensed under a single license, the licensee shall operate these buildings as a single integrated system as follows: (1) All buildings or portions of buildings under a single license shall be governed by a single governing body and under administrative control of a single administrator.(2) All hospital facilities operating under a single license shall have a single medical staff.(d) Reapplication shall be filed when a change of fifty percent (50%) or greater ownership occurs.(e) An application for license from a newly constructed hospital shall be obtained from the division and submitted after the physical plant plans have been approved under 410 IAC 15-1.5-8.
