Sec. 16.
(a) As used in this section, "augmentative communication device" means a device or system that compensates for the loss or impairment of a speech function due to a congenital condition, an acquired disability, or a progressive neurological disease. The term includes only that equipment used for the purpose of communication, including both electronic and nonelectronic devices.(b) As used in this section, "communication device" refers to an augmentative communication device.(c) Medicaid reimbursement is available for a communication device subject to the following: (1) The device must be ordered in writing by a medical doctor or doctor of osteopathy.(2) Prior authorization is required for a communication device. Documentation supporting that the device is medically necessary must be provided on, or attached to, the prior authorization request form submitted by the requesting practitioner. A clinical evaluation by a speech pathologist, substantiating the communication device is medically necessary, must be submitted as part of the prior authorization request.(d) Authorization of reimbursement for a communication device may be granted only upon satisfaction of all of the following:(1) Documentation must be presented that substantiates the member has demonstrated sufficient mental and physical capabilities to benefit from the use of the system.(2) Documentation must be presented that substantiates the member, in the absence of a communication device, cannot effectively make himself or herself understood by others in his or her communication environment.(3) Documentation must be presented that substantiates the member's medical condition is such that at least two (2) years of use of the device by the member can reasonably be expected.(4) Documentation must be presented that: (A) identifies all communication devices that would meet the member's communication needs, taking into account the physical and cognitive strengths and weaknesses of the member and the member's communication environment; and(B) recommends the least expensive communication device among those in clause (A). (5) If authorization is requested for a computer or computerized device, the intended use of the computer or computerized device must be compensation for loss or impairment of communication function.(e) Reimbursement for repair or replacement of a communication device is available in accordance with section 5 of this rule.(f) Subject to prior authorization, rehabilitation engineering services necessary to mount or make adjustments to a communication device are covered; and speech therapy services as medically necessary to aid the member in the effective use of a communication device are covered subject to this rule and 405 IAC 5-22.
