Section 405 IAC 5-16-1 - Providers eligible for reimbursement

Sec. 1.

Services provided to a member by:

(1) home health agencies;

(2) clinics;

(3) federally qualified health centers;

(4) free-standing surgical centers;

(5) therapy centers;

(6) rehabilitation centers; or

(7) other such facilities;

are covered subject to the limitations set out in this rule and 405 IAC 5-22.
