Section 329 IAC 12-7-8 - Duplicate certificate

Sec. 8.

In order to replace a certificate that has been lost or stolen, a certified operator must do the following:

(1) Submit a request for a duplicate certificate to the commissioner.

(2) Include a statement indicating the reason a duplicate certificate is needed.
