Sec. 4.5.
(a) A person may become certified as a facility specific operator (FSO) by a demonstration of proficiency:(1) through an examination;(2) based on completion of an approved training course; or(3) through another method approved by the commissioner.(b) A standardized examination prepared to reflect the duties and responsibilities required of each FSO water treatment plant and water distribution system certified operator shall be as follows:(1) Conducted at least annually.(2) Held at places and times established by the commissioner.(c) A person wishing to apply for water treatment plant or water distribution system FSO certification shall fulfill the following requirements:(1) Complete an application on a form approved by the commissioner that: (A) contains true and accurate information to the best of the applicant's knowledge; and(B) is free of omissions and misrepresentations, either of which may result in rejection of the application or revocation of any certificate previously granted.(2) Submit the following: (A) A completed application, with the necessary fee, to the commissioner.(B) Any additional information requested by the commissioner.Water Pollution Control Board; 327 IAC 8-12-4.5; filed Oct 24, 2006, 3:03 p.m.: 20061122-IR-327050255FRA
