Section 326 IAC 3-4-2 - Certification

Sec. 2.

Each report submitted under this article shall contain a certification of truth, accuracy, and completeness. This certification and any other certification required under this article shall state that, based on information and belief formed after reasonable inquiry, the statements and information in the document are true, accurate, and complete. Reports submitted under 326 IAC 3-8 shall meet the certification requirements of 326 IAC 2-7-4(f).
