Ill. Admin. Code tit. 77 § 682.150
Each corporation, partnership, trust, association or other entity engaging in the business oftesting,fitting, servicing,selecting,dispensing, selling, or offering for sale hearinginstrumentsat retail shall file, with the Department,prior to doing business in this State andby July 1 of eachcalendaryearthereafter, on forms prescribed by the Department, a list of all licensedhearinginstrumentdispensers/audiologists employed by it; the business name, address, county, and phone number; and the name of the owner and/or manager; and a statementattestingthat it complies with the Act and this Part and the regulations of the Federal Food and Drug Administration ( 21 CFR 801.420 et seq.) insofar as they are applicable. (Section 5 of the Act) The Department shall be notified, in writing, of any changes to the information provided.
