Section 1125.130 - Form of Petitionsa) All Petitions requesting the promulgation, amendment, or repeal of a rule must be submitted to the Department in writing. Each Petition shall contain the following: 1) the name of the individual or entity petitioning the Department, or if the individual is petitioning the Department on behalf of another individual or entity, then the name of both the Petitioner and the individual on whose behalf the Petition is being filed;2) the complete mailing address that the Department should use in directing its response to the Petition;3) a statement of the particular relevant interest of the petitioner or of the individual on whose behalf the Petition is being filed;4) the rule(s) that the Petitioner desires the Department to repeal or to revise, and, if the Petition is for an amendment, a clear statement of the desired change(s);5) if the Petition is for the adoption of a rule(s), a statement in as much detail as possible, of the proposed text and nature of the desired rule(s); and,6) a statement detailing the reasons and basis for the Petition, including any appropriate references.b) A model of the form of the Petition that requests the information required under subsection (a) is incorporated into this Part as Appendix A.
