Section 36.01.01.060 - FORM OF PLEADINGS

01.Form. Pleadings, except those filed on Board forms, submitted by a party and intended to be part of the record should be:

a. Double-spaced:

b. State the title of the pleading and the appeal number at the top of the cover page;

c. Include the name, mailing and street address, and if available, the telephone and FAX number of the person filing the document; and

d. Be signed by a qualified representative.
