Section 11.04.01.167 - FORM OF APPEAL01.Form of Appeal. An appeal must be in writing on a form prescribed by the Racing Commission and include:a. The name, address, telephone number and signature of the licensee making the appeal; andb. A statement of the basis for the appeal.02.Bond.
