A separate registration is required for each principal place of business or professional practice where the applicant manufactures, distributes, administers, dispenses, or stores controlled substances. Registrants are required to notify the narcotics enforcement division in writing of all non-registered office locations where the registrant only prescribes controlled substances but does not administer or dispense. Failure of a registrant to notify the department shall result in the automatic revocation of the registrant's controlled substance registration.
