Section 17-799-6 - Application process(a) A request for child care payments shall be submitted on an application form prescribed by the department.(b) The form shall be dated and signed by the applicant under penalty of law that all the information needed to establish eligibility for child care payments, as stated on the form, is true and correct.(c) The applicant shall be required to submit copies of documents for verification of the information provided to establish eligibility for the program.(d) The date of application shall be the date the signed and dated application form is received by the department.(e) An application received outside of an open application period shall be subject to 17-799-18(a)(2).[Eff 2/09/14] (Auth: HRS § 346-14, HRS § 346-181) (Imp: HRS § 346-14, HRS § 346-181)
