A new application form need not be completed when the applicant, whose application for financial assistance was denied or discontinued, reapplies for financial assistance within one hundred twenty days from the date of the previous application. The application form shall be reviewed with the applicant, and any changes shall be recorded on the application form and other appropriate department forms. The new date of application shall be entered on the application form. The reapplication shall be processed in the same manner as a new application.
