Section 17-1740.1-18 - Billing(a) Each FQHC or RHC shall complete and submit to the department's fiscal agent the appropriate claim form for any covered item or service, regardless of whether a claims-based interim payment will result.(b) The claims shall either be on forms provided by the department or in a format that the department has indicated in advance is acceptable.Haw.
