Section 17-1722-38 - Application for payment of medical services(a) A pensioner or a surviving spouse shall sign an application form as a condition for approval of the application.(b) A relative, friend, or a designee may be asked to complete and sign the application form on behalf of a pensioner or spouse when the pensioner or spouse is determined by the department to be unable to do so.Haw.
