A forfeited license may be restored within one year after the date of expiration by filing a renewal application along with any arrest or conviction information and any information on any psychiatric or psychological disorder for which treatment is being received, and paying the renewal fees plus a penalty fee. A person who fails to restore a forfeited license within one year of the date of expiration shall be considered a new applicant and shall satisfy all requirements for licensure in effect at the time the person reapplies for a license.
