Any person seeking registration to practice as a massage apprentice must apply on a form or forms prescribed by the board, and shall submit a letter of agreement from the sponsoring massage therapist indicating the starting date of the practical phase of the apprenticeship program and the number of hours per week the applicant will be directly supervised by the sponsoring massage therapist and training program to be subject to board's approval.
