The fees for the board of examiners in optometry shall be as provided in this section.
| (1) Application fee | $ 15 |
| (2) Original license fee | $ 25 |
| (3) License fees: | |
| (A) First year of the biennium (original license fee, plus one-half of the biennial renewal fee) | $ 75 |
| (B) Second year of the biennium (original license fee) | $ 25 |
| (4) NBEO examination and other examination related fees as determined by agreement between the board and a professional testing service | |
| (5) Biennial renewal fee for optometry license | $ 100 |
| (6) Recordkeeping fee | $ 20 |
| (7) Restoration of forfeited optometric license shall be the biennial renewal fee, penalty fee of $50, in addition to the recordkeeping fee for each biennium the license was forfeited. | |
| (8) Reinstatement of suspended license shall be the penalty fee of $50, in addition to all delinquent fees. | |
| (9) Compliance resolution fund: | |
| (A) Compliance resolution fund fee | $ 35 |
| (B) Annual compliance resolution fund fee | $ 35 |
| (C) Biennial compliance resolution fund fee | $ 70 |
