The fees for advanced practice registered nurses with prescriptive authority shall be as provided in this section:
| (1) Application fee | $ 50 |
| (2) Original granting of prescriptive authority fee | $ 20 |
| (3) Original fees: | |
| (A) First year of the biennium (original granting fee, plus one-half of the biennial renewal fee | $ 40 |
| (B) Second year of the biennium (original granting fee | $ 20 |
| (4) Biennial renewal fee | $ 40 |
| (5) Restoration fee for failure to renew prescriptive authority shall be the penalty of $30, in addition to the biennial renewal fee. | |
| (6) Modification, termination or reestablishment of prescriptive authority relationship processing fee | $ 50 |
| (7) Compliance resolution fund: | |
| (A) Compliance resolution fund fee | $ 35 |
| (B) Annual compliance resolution fund fee | $ 35 |
| (C) Biennial compliance resolution fund fee | $ 70 |
