The fees for commercial employment agencies and principal agents shall be as provided in this section:
| (1) Application fees: | |
| (A) Agencies and branch offices | $ 50 |
| (B) Principal agents | $ 25 |
| (2) Examination and other examination related fees as determined by agreement between the department and a professional testing service | |
| (3) Original license fees: | |
| (A) Agencies and branch offices | $ 60 |
| (B) Principal agents | $ 25 |
| (4) License fees: | |
| (A) Agencies and branch offices: | |
| (i) First year of biennium (original license fee, plus one-half of the biennial renewal fee) | $ 110 |
| (ii) Second year of biennium (original license fee) | $ 60 |
| (B) Principal agents: | |
| (i) First year of biennium (original license fee, plus one-half of the biennial renewal fee) | $ 50 |
| (ii) Second year of biennium (original license fee) | $ 25 |
| (5) Renewal fees: | |
| (A) Agencies and branch offices | $ 100 |
| (B) Principal agents | $ 50 |
| (6) Restoration fee for failure to renew shall be a penalty fee of | $ 80 |
| (7) Duplicate identification cards: | |
| (A) Agencies and branch offices | $ 10 |
| (B) Principal agents | $ 10 |
| (8) Duplicate certificate fee | $ 10 |
| (9) Inactive fee for principal agent and agency | $ 10 |
