Section 16-101-35 - Application form

Application for license by a professional corporation shall be filed in a form as provided for by the board. If the corporation is duly organized and existing pursuant to chapter 416, HRS, and from the application it appears that the requirements for registration have been met, the board shall issue, upon payment of the registration fee, a certificate of registration.
