The operator of any well proposed to be abandoned shall file with the chairperson an application for permit to abandon, prior to the abandonment. The operator's proposed plans for abandonment shall be subject to approval and revision prior to the issuance of a permit by the chairperson. Each application to abandon a well shall be accompanied by a non-refundable filing fee of $100.
