When the stamping on a pressure retaining item becomes indistinct or the nameplate is lost, illegible, or detached, but traceability to the original pressure retaining item is still possible, the inspector shall instruct the owner or user to have the stamped data replaced, following the requirements of the original code of construction, except as modified herein. An application to re-stamp or replace nameplates shall be made to the department using the National Board Replacement of Stamped Data Form (NB-136), and must be accompanied by proof of the original stamping and other such data, as is available, and the remittance of the appropriate fee for each pressure retaining item subject to this part as per the schedule in Exhibit A, titled, "Installation, Repair or Alteration Permit Fees, and Licensure, Examination, and Registration Fees", dated October 1, 2023, which is made part of this chapter and located at the end of this chapter, and inspection fee for each pressure retaining item subject to this part as per the schedule in Exhibit B, titled, "Internal & External Inspection Fees", dated October 1, 2023, which is made a part of this chapter and located at the end of this chapter. Additional fees may apply if the department is requested to witness the attachment of the replacement nameplate. When that traceability cannot be established, the department shall be contacted.
