Section 11-800-28 - Reassessment(a) The case management agency shall conduct face-to-face reassessments every six months or sooner, as appropriate, and shall use standardized assessment tools to reevaluate the client's: (1) Diagnosis, health, functional, psychosocial, financial, and environmental needs; and(2) Compliance with the client's service plan.(b) The case management agency shall have the client's physician re-certify the client's level of care on an annual basis or more frequently as needed.
