Section 11-504-55 - Course Certificates The training provider shall issue certificates to students who successfully pass the examination at the end of an initial or refresher training course. The certificate shall include the following information:
(1) A unique, identifying number for each student;(2) The name, address, and phone number of the training provider;(3) The student's name and social security number;(4) The name of the course for which that student has received certification;(5) The date(s) of the course and the examination;(6) The expiration date of the certificate, limited to one year from the date of the examination; and(7) A statement specifying that the training course meets all requirements of the Asbestos Model Accreditation Plan and that the training provider is accredited to provide training within the State of Hawaii.[Eff ] (Auth: HRS § 342P-41) (Imp: 40 CFR Part 763, Appendix C(1)(C))
