Section 11-160-43.07 - Registration card; content

A registration card for a qualifying out-of-state patient or a caregiver of a qualifying out-of-state patient shall, at a minimum, contain:

(1) The name and date of birth of the qualifying out-of-state patient;

(2) The name and date of birth of the caregiver of a qualifying out-of-state patient;

(3) The registration identification number; and

(4) The registration card's effective and expiration dates.
