Rule 750-8-.01 - Change of Name or Address(1) Change of Name. The holder of a certificate shall notify the Board office, in writing within 30 days of a change in their name and must accompany such notice with a certified copy of the marriage certificate, court order, or other documentation of a legal name change.(2) Change of Address.
