Rule 560-10-30-.04 - Agreement and License - General(1) Upon approval of the application, the Commissioner shall deliver to the applicant an agreement setting forth the terms and conditions for use of all information provided by the Department.(2) Applicant shall have no later than forty-five (45) days from the date of mailing of the agreement by the Department to respond. Failure to respond later than forty-five (45) days from the date of mailing may be deemed a withdrawal of the application. Should the applicant wish to reapply for a license, a new application will have to be submitted.(3) Upon receipt of a signed agreement from the applicant and approval by the Commissioner, the Department shall issue a license along with a fully executed copy of the agreement to the applicant.Ga.
