Rule 511-1-3-.11 - Abstraction of Documentary Evidence(1) The State Registrar, or his or her designee, shall abstract onto the amended birth certificate form or the delayed certificate of birth a description of each document submitted to support the alleged amendment or the facts shown on the delayed birth certificate. This description shall include the following: (a) The title or description of the document;(b) The name and address of the custodian, if the document is an original or certified copy of a record, or a signed statement from the custodian;(c) The date of the original filing of the document being abstracted and,(d) All birth facts required by these Rules, contained in each document accepted as evidence.(2) All documents submitted in support of an amendment or for the registration of a delayed birth certificate shall be returned to the applicant after review.Ga.
