Original Rule entitled "Practice, Responsibility, Functions of Licensed Practical Nurses" was filed on February 5, 1982; effective February 25, 1982, as 400-2-.11, and by filing of April 22, 1983; effective May 12, 1983, was renumbered as Rule 400-2-.12.

Repealed: New Rule entitled "Licensee Name and Address Changes. Lost Licenses" adopted. F. May 19, 1994; eff. June 8, 1994.
