Health insurance policies and plans, except limited benefit insurance policies, shall provide coverage for the following pharmacologic agents when prescribed by a physician licensed to practice medicine pursuant to Title 43:
(1) Insulin of each class approved by the federal Food and Drug Administration (FDA) including formulations available either in a vial or cartridge;(2) Prescription insulin of each class approved by the federal Food and Drug Administration (FDA) including formulations available either in a vial or cartridge formulation;(3) Prescription oral medications of each class approved by the federal Food and Drug Adminstration (FDA) for the management of diabetes;(4) Oral products approved by the federal Food and Drug Administration (FDA) for the management of diabetes;(6) Pharmacologic agents approved by the federal Food and Drug Administration (FDA) for the management of diabetes and its complications.Ga.
