Rule 120-2-45-.12 - License and Renewal Fees(1) License Fee. A license fee of $600 shall be paid at the time of the filing of an Application for Certificate of Authority, for a Captive Insurance Company.
(2) Renewal Fee. A renewal fee of $500 shall be due for each year after initial licensure of a company.
