Rule 120-2-2-.20 - Initiation of Adjudication1. An adjudication is initiated when a party files a petition.2. The petition may be sent to the Department by either:a. Mail: ATTN: Administrative Procedure Division, Department of Insurance

b. Email: AdminProc@oci.ga.gov3. The petition must contain the following:a. addressed to the Commissioner;b. signed by the petitioner or his/her counsel;c. a brief statement of the nature of the proceeding,d. the identity of known parties,e. the jurisdiction under which the adjudication is initiated,f. a brief statement of the general allegations of fact and the issues to be adjudicated,g. the legal authority that constitutes a basis for the adjudication, andh. a brief statement of the nature of the relief sought.Ga.
