Section 69O-238.001 - Application and Renewal
(1) An applicant for initial registration as a pharmacy benefit manager shall submit the following:
(a) A nonrefundable fee of $5.00;
(b) A copy of the applicant's corporate charter, articles of incorporation, or other charter document submitted electronically via the Office's iApply system at https://www.floir.com/iportal; and
(c) A completed Form OIR-C1-2209, Registration Form for Pharmacy Benefit Managers, effective 01/19, hereby incorporated by reference and available at http://www.flrules.org/Gateway/reference.asp?No=Ref-10321, submitted electronically via the Office's iApply system at https://www.floir.com/iportal.
(2) Each registrant renewing its registration shall submit a nonrefundable fee of $5.00 and a completed Form OIR-C1-2209 submitted electronically via the Office's iApply system at https://www.floir.com/iportal.
