Section 69O-161.006 - Use of Pharmacy Claim Form

All companies writing health insurance shall accept the Universal Pharmacy Claim Form (Form DAH) (2/89) and Form HCFA 1500, which are hereby incorporated and made part of this rule chapter by reference.
